Healthsense PCN Access Plan
Over the last year, our PCN practices have continued to improve how patients access care by strengthening telephone systems, expanding online consultation services, and improving care navigation processes across the network. Practices have used patient feedback and access data to better understand demand and adapt services accordingly, supporting more timely responses and improved access to care. There has also been a continued focus on supporting continuity of care for patients with more complex or ongoing health needs.
Our ongoing access approach is based on providing fair, responsive, and easy-to-navigate care through a combination of online, telephone, and face-to-face access routes. Practices will continue to develop triage and care navigation systems, while listening to patient feedback through surveys, participation groups, and community engagement to help shape future improvements.
Help us help you get the treatment you need
While everyone is being told to stay at home, it can be hard to know what to do if you’re unwell.

- For urgent medical help – use the NHS 111 online service, or call 111 if you’re unable to get help online.
- For life-threatening emergencies – call 999 for an ambulance.
